Abstract

Telehealth supported care coordination for patients with chronic diseases has been established for more than 10 years. New requirements and challenges for the development of a modern telemedical platform are emerging as organizations attempt large-scale implementations and the integration of telehealth in a variety of care settings. This paper describes a newly developed telehealth system that is already deployed commercially along with future challenges facing this system.
